Committed to implement a new Transforming Communities Initiative that will provide grants for communities to reduce tobacco use, lower childhood obesity rates, support early childhood health and wellness and improve access to nutrition and physical activity opportunities.

    Commitment Element

    Provide six identified communities up to $500,000 annually for up to 5 years to work with community partners to reduce tobacco use and lower obesity rates.

    Progress To Date

    Eight organizations received a total of $3.1 million in 2019 to work on projects to improve community health and lower obesity rates. Projects focused on food security, tobacco use reduction, trauma and resilience, community assessment, coalition building and engagement, access to care, breastfeeding support, creating healthier and safer school environments, creating healthier childcare environments, improving the built environment, and increasing physical activity. Since 2017, the eight organizations have received a total of $8.9 million.

    Commitment Element

    Make $40 million available in Socially Responsible Investments in housing, early childhood health and wellness, and food access issues.

    Progress To Date

    In 2019, Trinity Health provided $3.5 million to Bethlehem Haven in Pittsburg to build a 38-unit affordable housing development for individuals and families making 50% or less of the area median income. It also provided $1 million to RPM Group to support the development of Patriot Village II, a 70-unit affordable housing development in Trenton, NJ. Since 2017, Trinity Health has made $6 million available in Socially Responsible Investments in housing, early childhood health and wellness, and food access issues.

    Maximize the impact of these investments by assisting communities with identifying and securing up to $22.5 million in cash match awards.

    Progress To Date

    In 2019, the eight organizations that received grant awards to improve community health and lower obesity rates identified and secured $2,678.988 in cash match. Total cash match secured since 2017 is $6,917,488, 31% of the cash match goal.
